The beginnings of the organized provision of health (then social) insurance in the territory of the Republic of Croatia appear in the twenties of the last century, and until today health insurance holders have undergone numerous reforms both in the name itself and in their organizational structure, but always with one single goal: the implementation of insurance workers and taking care of their insured.

During the Kingdom of Serbs, Croats and Slovenes, the National Treasury for the support of sick workers and for insurance against accidents in Zagreb, i.e. the Central Office for Workers' Insurance in Zagreb , which was established by the Workers' Insurance Act on May 14, 1922, and its first the director was the distinguished Vilim Bukšeg (1874-1924). Show more Insurance in Zagreb was responsible for workers' social insurance for the entire country at that time (the Kingdom of Serbs, Croats and Slovenes, later the Kingdom of Yugoslavia) in the period from 1922 to 1941.

During the Independent State of Croatia, the Central Office for Workers' Insurance changed its name and became the Workers' Insurance Center (with branches) , and the Health Center for State Employees was also established.

After the end of II. after World War II, there was a merger of all existing holders of social insurance at that time, regardless of the branch of insurance and the coverage of persons for whom the insurance was provided. Instead of the Center for Workers' Insurance, the Central Institute for Social Insurance and Land Institutes for Social Insurance was established, which operated under that name until January 1, 1947, when the entire operation was taken over by the State Institute for Social Insurance .

At the beginning of the fifties of the last century, there was a new reorganization of social insurance holders, so that since 1952 the Institute for Social Insurance of the People's Republic of Croatia and Kotarski zavodi za sociali osiguranje (city) have been operating, in which insured persons manage the institutes through their representatives, and for the purpose of insurance of the insurance funds establishes a social insurance fund. On the basis of this new organization, within the Social Insurance Institute of the Republic of Croatia, the following were established: the Pension Insurance Fund and children's allowance as the Republic Fund and the Health Insurance Fund at the level of district and city institutes.

With the reorganization of the former Institute, the Republic Institute for Social Insurance and communal social insurance institutes operate from January 1, 1963 , as part of which health and pension insurance is provided.

Such an organization remained until 1971, when pension and disability insurance were separated from social insurance, and the Workers' Health Insurance Association and the Farmers' Health Insurance Association were organized as health insurance carriers (applied from January 1, 1971), and Temporary health insurance associations for workers and temporary associations for health insurance for farmers (applied since January 1, 1972), which continue the work of the previous municipal social insurance associations for workers and farmers.

Since 1974, health insurance has been carried out in a self-governing community of interest, established for the entire area of ​​Croatia, under the name Self-governing interest community of health insurance and health care , and they operated that way until September 1, 1990, when the aim was to fulfill the personal and common needs of workers and of other working people and citizens when these needs and interests are at the same time the needs and interests of society and for the purpose of realizing the rights guaranteed by the constitution and the law in the field of health and health care, the Republic of Health Insurance Fund of Croatia is established .

The 1990s were marked by numerous normative reforms as well as war events, and all of this had an impact on reforms in the field of health care and health insurance, so that since August 21, 1993, the holder of health insurance is the Croatian Health Insurance Institute , which is took over operations from the Republic Health Insurance and Healthcare Fund of Croatia, which until today operates under the aforementioned name with the aim of providing all insured persons with the rights and obligations from compulsory health insurance based on the principles of reciprocity, solidarity and equality.
